You can use the convenient online Consumer Toolkit around the clock to review your eligibility and benefits information, keep tabs on the status of your processed claims and manage other aspects of your enrollment.
Delta Dental can accept and process PHS ADDP claims submitted in any legible format. For your convenience, we have provided a standard claim form for you or your dentist to complete. The Consumer Toolkit provides PHS ADDP enrollees with convenient access to track and monitor their claims-related activity.
Claims can be submitted electronically by your dentist as well as in hardcopy via U.S. mail. Contact us if you need assistance in completing your claim or for the address to submit your claim.
Overseas claims for the PHS ADDP are processed and paid according to Delta Dental's established policies. Overseas dental providers are not required to submit standard claim forms or utilize the American Dental Association (ADA) Current Dental Terminology codes but should provide you with a detailed receipt for services provided. Your overseas claims will be translated and cross-referenced to the appropriate corresponding ADA CDT codes by Delta Dental. Delta Dental will reimburse overseas enrollees in U.S. dollars using the exchange rate in effect on the date of service.

Delta Dental's grievance process is designed to assist PHS ADDP enrollees with a benefit problem or other issue that cannot be resolved satisfactorily by another means. Any PHS ADDP enrollee who has a grievance against Delta Dental for any matter arising from his or her enrollment may file a written complaint. The grievance must be made in writing and should be directed to Delta Dental at the address listed on the Contact Us page of this website.
